Please arrive 5 to 10 minutes before your scheduled appointment and bring the following items:
Proof of identification
such as a driver license
Social Security Cards for you, your spouse and dependents or Individual Taxpayer Identification Number (ITIN) for you, your spouse and dependents
Birth dates for you, your spouse and dependents
Wage and earning statement(s)
such as Form W-2, 1099’s, 1098’s and all other tax forms you receive
Total paid for day care and provider's tax ID
A copy of last year's Federal and State returns if available
Proof of bank account routing numbers and account numbers for Direct Deposit, such as a blank check
Proof of health insurance
, Forms 1095-A, B or C
To file taxes electronically on a married filing joint tax return, both spouses must be present to sign the required forms
If you are missing a form that you will need to file your taxes, please call the IRS helpline toll-free at 1-800-829-1040 for information on what you need to do or how to obtain replacement documents
You will be asked to fill out a detailed
Intake Sheet (IRS Form 13614c)
. You can speed things up by filling this out at home (first, second, and third pages), printing, and bringing it with you! It only takes 5 minutes to fill out!
